End‐of‐Life care

JCECC: “Life Rainbow” End‐of‐Life Care Services has kicked off in January. It is a pilot project with an integrative model of End‐of‐Life care for late‐stage chronic patients residing in the community for enhancing their quality of life using a collaborative partnership approach.
